What you'd do

Make clinical decisions for heart failure, diabetes, and hypertension
Manage a panel of patients with evidence-based guidelines
Collaborate with physicians and RNs to optimize treatment plans

What they're looking for

Active Compact Multi-State RN license
Active NP licensure in at least two specified states
5+ years experience treating chronic diseases
3+ years of cardiology experience focused on heart failure
